Individuals can register in advance for one of the events listed below to save their appointment time. You should bring your health insurance information and ID to your appointment. Vaccines are covered by most major insurance plans and a limited number of no-cost shots will be available for people without insurance.

• Photocopy of your Driver's License.
• Photocopy of the front and back of your insurance card.
• Staff and Members welcomed.
