Youth Program Forms

Forms parents may need for participation in YMCA Youth Programs or Orange County school programs. 

Medication and Sunscreen

The Medication Distribution & Sunscreen Application form is used when a camper needs medication administered during YMCA program hours and/or when a child needs assistance in applying sunscreen or insect repellent. One form per child/ per medication.

The YMCA Youth Information Form is used to register a child for youth programs only when online registration is unavailable. A Spanish version is available also.
